Futsal Coach Registration

Coaches, parents will register their own children individulaly this year.  This is an attmept to streamiline the registrtaion process for everyone involved.  Each parent will also be repsonsible for paying their own children's registration fees at the time of regsitration.  

What we need from you is as follows:

1.  Coach's name

2. Coach's phone number 

3. Team contact name 

4. Team contact phone number (cell please for texting)

5. Team location

6. Team name 

7. Each team player's name 

8. Are you ordering shirts?

9. If so, list three color choices
